SN Services has staff based on site 24/7 to provide planned and unplanned daily supports and nursing services.   

If you choose to utilise SN Services you will be assured that their team are highly trained in complex care requirements.  They also understand they are in your home, you are not in their workplace.  With a highly client-focused approach, your every day life is delicately balanced with your clinical needs.

SN Service is also highly committed to your therapy programme, and will provide a service that helps you and your loved ones adapt to using equipment and undertake household and personal care activities in a supportive environment as part of your overall transition programme from hospital.

If you are new to the NDIS you will probably have received a crash course in how NDIS funding packages work.  There are 2 principles to be aware of.  You will have:

1. Your own individual funding package with a budget designed for your specific needs, which may change from time to time as your needs and circumstances change; and
2. Choice and control to select your preferred service provider.

With your individualised funding package, you can choose to enter into a Service Agreement with one or all of the collaboration partners OR select an alternative service provider(s).  Your choice.  You can also make changes to your team at any time if the fit isn't right for you.

The NDIS typically does not fund accommodation except in a few circumstances.   These circumstances are :

1. Medium Term Accommodation - for durations up to 90 days for people leaving hospitals or requiring temporary accommodation while waiting for access to their home.   You can read more about our MTA offering here.*
2. Specialist Disability Accommodation - this is an annual allowance granted to people who meet the NDIS criteria for housing to meet their needs specific to their disability.  The NDIS allocates funds in your plan to subsidise your rent.  We currently have a couple of remaining vacancies for SDA accommodation.   See our SDA page for more details.
3. Short Term Accommodation - this funding is available on a case-by-case basis for up to 28 days those who have carers that may need a break.  It may be possible for you to book short-term stays with us and benefit from the services on offer by our collaborators, or bring your own supports.

*From 3 October 2024 Medium Term Accommodation funding must be stated in your funding plan.  Your hospital NDIS Liaison, Social Worker, Support Coordinator, NDIS Planner or LAC should be made aware as early as possible that you will need this funding stated in your plan to be eligible. 

For both MTA and SDA, there will be modest, affordable out-of-pocket rental expenses and utilities costs.

Your funding plan will provide an allowance to meet your needs that relate to your disability.  If you are new to the NDIS, this process will begin in hospital with support from your social worker, NDIS liaison and possibly a support coordinator.   If you have found us, and do not yet have a funding plan in place we highly recommend you reach out to our team to consult with them about their services to help build a plan with your hospital team that allows you to transition from hospital spending time at 451 Beach as a stepping stone to your permanent home.

The uncomfortable reality is the NDIS needs funding efficiencies.  By choosing this transitional service you get the benefit of your own brand new apartment to live in rather than a shared living arrangement, while sharing supports with others.  In fact, your time at our service will likely be more affordable for the NDIS while you are with us than when you return home and can't share supports.  We are here to make sure your stay is affordable.  The gains you make during your stay will hopefully deliver savings in your daily support needs long term as you launch better equipped for the daily challenges ahead.
